Your doctor writes "TFT" on a slip, you go to a lab, and you are quoted anywhere from Rs 655 to Rs 5,000 for what sounds like the same blood test. That range is not a scam. It is three different tests with similar names, priced differently at labs with different equipment and different discount schemes. Knowing which one you actually need saves you a few thousand rupees, and knowing how to read the number on the report saves you weeks of worrying about a result that was never abnormal. Here is what thyroid testing costs in Pakistan in August 2026, which test to ask for, and what the numbers mean.
TSH, T3, T4: which one do you need?
The thyroid gland sits at the front of your neck and sets the pace of your metabolism. Your pituitary gland monitors it and sends out thyroid stimulating hormone, TSH, to push it harder when thyroid output drops. That relationship is why TSH is the test that matters most.

The counterintuitive part: TSH moves opposite to thyroid function. A high TSH means your pituitary is shouting at an underactive thyroid, so high TSH points to hypothyroidism. A low TSH usually means the thyroid is producing too much, so hyperthyroidism. People read a high number and assume overactive. It is the other way round.
For a first check, TSH alone is enough for most people, and it is the cheapest option. Paying for a full profile before you know whether TSH is even off is how a Rs 1,200 test becomes a Rs 4,000 one.

Three practical notes on those numbers. Booking through an aggregator like Dawaai, InstaCare or Healthwire is usually 10 to 30 percent cheaper than walking into the same lab's collection point, because the discount is baked into the platform rate. Home sample collection is free at most large labs now, so there is no reason to sit in a queue for a routine blood draw. How to prepare, and when to give the sample
Thyroid tests do not need fasting. You can eat normally. Three things do matter.
- Timing. TSH follows a daily rhythm and is highest in the early morning, dropping through the afternoon. A borderline TSH at 8am can look normal at 3pm. If you are being retested to track a trend, give the sample at roughly the same time of day each time.
- Your thyroid medicine. If you already take levothyroxine, take the sample before your morning dose, not after. A dose taken an hour earlier will lift your T4 reading and confuse the picture.
- Biotin supplements. This one catches a lot of people. High dose biotin, the kind in hair and nail supplements, interferes with the lab method used for thyroid assays and can produce falsely high T4 and falsely low TSH, a pattern that looks exactly like an overactive thyroid. Reading your report without panicking
Reference ranges printed on Pakistani lab reports are broadly consistent, though they differ slightly by lab and machine. Commonly used adult ranges run from about 0.4 to 4.1 mU/L for TSH, with some labs going up to 5.2 for older adults. Your report will print its own range on the right hand column. Use that one, not one you found online.

One number slightly outside the range is not a diagnosis. TSH fluctuates with acute illness, sleep loss, pregnancy, and even the time you gave the sample. A single mildly raised TSH is normally repeated after six to twelve weeks before anyone starts you on anything. Do not begin a medicine on the strength of one borderline report from a lab you walked into on a whim.
Pregnancy is the exception where speed matters. The acceptable TSH range in pregnancy is tighter, commonly around 0.6 to 3.4 mU/L in the first trimester, because thyroid hormone matters for the baby's brain development. When a thyroid test is worth the money
Fatigue alone is a weak reason to test, because almost everything causes fatigue. The picture that genuinely warrants a TSH looks like several of these together: unexplained weight change in either direction, cold or heat intolerance that is new, hair thinning across the scalp, a puffy face, constipation or a racing heart, periods that have become much heavier or much lighter, or a visible swelling at the base of the neck.
Thyroid disease is more common in women, and risk rises after pregnancy and around menopause. It also runs in families, so if your mother or sister is on thyroxine, your threshold for testing should be lower. Iron deficiency and vitamin B12 deficiency produce a very similar tiredness and hair fall picture and are far more common in Pakistani women, which is why doctors often order them together. What treatment costs, if you need it
The reassuring part of a hypothyroidism diagnosis is that the treatment is old, well understood and cheap. Levothyroxine is sold in Pakistan under names including Thyronorm and Thyroxine. A pack of 100 tablets of Thyronorm 50mcg lists at around Rs 267, often discounted to about Rs 240 at online pharmacies. That is a few rupees a day.
Levothyroxine is a prescription medicine and the dose depends entirely on your weight, age, test results and other conditions, so it must come from your doctor. Ask your pharmacist about how to take it, particularly the timing around food, calcium and iron supplements, which interfere with absorption. Never adjust the dose yourself because you feel better or worse, and never take someone else's leftover strip because your symptoms sound similar to theirs.
