What whitening toothpaste can and cannot do

Cleveland Clinic splits tooth discolouration into two types. Extrinsic stains sit on the outer enamel and mostly come from food and drink. Intrinsic discolouration starts inside the tooth, in the dentin, and comes from things like dental trauma or some medicines (Cleveland Clinic: tooth discoloration).

Whitening toothpaste works on the first type. It polishes and lifts surface stains during normal brushing. It does not bleach the tooth from inside, so it will not change the colour you were born with or a tooth that has gone grey after an injury.

The evidence for that surface effect is decent. A 2025 systematic review pooled 14 randomised trials and found whitening toothpastes reduced both the area and the intensity of surface stains compared with regular toothpaste, with the authors backing their use for extrinsic stains (systematic review on PMC). They also noted the studies varied a lot and that long-term effects need more research.

Why teeth go yellow in the first place

Cleveland Clinic lists dark drinks and foods (tea, coffee, berries, soy sauce), smoking and tobacco, and poor oral hygiene as avoidable causes, because stains cling to plaque. Ageing is the big unavoidable one: enamel thins over the years and shows more of the yellowish dentin underneath. Genetics also set your natural shade.

The same page gives a rough colour guide. Yellow stains usually come from dark foods and drinks, brown often relates to smoking or chewing tobacco, and grey can mean the nerve inside a tooth has died. Brown stains with small holes, or black spots, point to decay, which is a job for a dentist, not a toothpaste.

Colgate Optic White Lasting White

Optic White is Colgate's whitening line and Lasting White is the everyday paste in it. Our listing describes it as polishing off surface stains and leaving a coating that helps stop new stains building up between brushes. It suits adults and children over 12. Our FAQ is clear that most people notice surface stains lifting within a couple of weeks of twice-daily use, and that it will not change your natural tooth shade.

Crest 3D White Advanced

This is the full-size 147g tube from the USA. Our listing says it removes surface stains from tea, coffee and smoking and carries fluoride for cavity protection. The bigger tube is the main reason to choose it: one tube covers a couple sharing a bathroom for a good while. It is a different product from the cheap Crest Cavity Protection you may see in local shops.

Pearl Drops: Strong Polished and Smokers

Pearl Drops is a UK tooth polish brand. Strong Polished is its daily whitening paste, which our listing describes as low abrasion with enamel care. The pack claims up to four shades whiter by removing surface stains, which means stains from chai, coffee and food, not a change in natural colour.

Smokers comes in an upright flip-cap bottle and is made for heavy surface stains from tobacco, though it works the same way on sheesha, strong chai and paan stains. Its label says adults only.

Arm & Hammer Advance White

Arm & Hammer pastes are built around baking soda, which cleans and lifts surface stains. Our listing lists sodium bicarbonate and sodium fluoride. The taste is slightly salty, and most people get used to it within a few days.

If whitening toothpaste makes your teeth ache

Some people find standard whitening pastes leave their teeth twinging with cold water or ice cream. If that is you, switch to a whitening paste made for sensitive teeth rather than giving up. Sensodyne Pronamel Gentle Whitening 75ml (Rs 1,600) is low in abrasion and contains potassium nitrate and sodium fluoride at 1450 ppm, according to our listing.

What about charcoal toothpaste?

Still, it is fair to tell you that Cleveland Clinic lists activated charcoal and citric acid among the ingredients to avoid in home whitening products, because some over-the-counter whiteners can damage enamel. How to use a whitening toothpaste properly

The routine is the same as any fluoride toothpaste. The NHS advises brushing twice a day for about 2 minutes, with one of those times last thing before bed (NHS: how to keep your teeth clean).

  1. Check the fluoride. The NHS says adults should use a toothpaste with at least 1,350 ppm fluoride. The amount is printed on the pack.
  2. Brush every surface. Inside, outside and chewing surfaces of every tooth. A soft or medium brush is fine for most people.
  3. Spit, do not rinse. The NHS advises spitting out the excess and not rinsing with water straight away, so the fluoride stays on your teeth.
  4. Wait after acidic food. Cleveland Clinic suggests waiting about an hour after citrus, soda or sweets before you brush, because acid softens enamel.

Pair the paste with a decent brush. An electric brush is not required, but the NHS notes some people find it easier to clean thoroughly with one; our electric toothbrush guide compares the options.

Habits that keep the results

A whitening paste works best when fewer new stains land on your teeth. Cleveland Clinic's prevention list is practical for a Pakistani routine:

  • Cut back on how much tea, coffee and cola you drink, or drink them in one sitting instead of sipping all day.
  • Drink water and rinse your mouth after chai or coffee.
  • Stop smoking and chewing tobacco or paan with tobacco.
  • Keep up routine dental cleanings, which remove hardened deposits a toothpaste cannot.

When a toothpaste is not enough

If your teeth are naturally darker, or one tooth has changed colour, a toothpaste will not fix it. The NHS describes professional whitening as a gel used by a dental professional, either with trays at home for around 2 to 6 weeks or with a laser in the clinic (NHS: teeth whitening). It warns that kits from salons, pharmacies or online may not work as well and can harm teeth and gums.

The NHS also says professional whitening is not done for anyone 17 or younger, and is not recommended during pregnancy or breastfeeding, or on crowns, dentures and implants, since it only lightens natural teeth.
